PNLQ: Peripheral Neuropathic Syndromes Lower Quarter

Purpose
This two-day lab course will present concepts of differential diagnosis and treatment of patients with lower extremity radicular symptoms, pioneered by Bob Elvey PT, G.D.M.T.  Emphasis will be placed on precise physical evaluation of neural, articular, and muscular tissues in order to allow for the specific treatment of each. In particular, this course will cover the differentiation of pain sources within the lumbar spine, the piriformis triangle, the hip joint, and the peripheral nerves themselves. Specific evaluation of the lumbar and sacral plexus will be demonstrated. This course will cover well-recognized Orthopedic, Industrial, and Sports Medicine disorders of the lower quarter.  These are including but not limited to piriformis syndromes, lumbar stenosis, "chronic hamstring tears," “failed back syndrome", nerve root adhesions, peroneal and tarsal tunnel syndromes, chronic plantar fasciitis etc. Differences between neuropathic and neurogenic pain syndromes including a review of new research into the treatment of CRPS I and CRPS II will be presented.  Real patient examinations, videos, case presentations and supervised labs will be used to further allow for application of the material into the participants daily practice.

Objectives
The participant will be able to identify:
~The mechanisms by which neural tissue may in itself be a pain source.
~The role neural tissue may play in conditions of the lower quarter commonly evaluated and treated by physical therapists.
~The pathophysiology of neuropathic pain and the resultant widespread influence on the musculoskeletal system, patterned movement, and overall lower quarter function.
~Differential diagnosis and specific treatment progressions in lower quarter neuropathic syndromes. 

The participant will develop the manual and observational skills to:
~Assess the six specific and sensitive tests for ruling in or out neuropathic syndromes as the primary or secondary driver of a patient’s dysfunction.
~Assess the specific pain driver location for application of techniques for each individual patient.
~Perform the appropriate manual treatment and home program development to decrease pain and restore function in lower quarter neuropathic patients.
~Perform, specific to each patient, re-evaluation techniques to insure efficacy of treatment protocols. 

By the completion of the course, participant will demonstrate the ability to rule in and rule out neuropathic pain as the causation of patient’s pain, pattern losses, and functional disability.  Participants will be able to describe the complexity and far reaching consequences peripheral nerve irritability can have on musculoskeletal tissues and function.  Participants will be able to identify standard orthopedic maladies of the lower quarter that can have their origins in neurogenic inflammation.  Participants will be able to describe the specific pathophysiological changes that lead to these syndromes and how the treatment skill learned in this seminar helps to reverse this pathology.

By the end of the course, participants will demonstrate the skills and knowledge base to integrate both the evaluation skills and the treatment protocols into their normal protocols.  Participants will be able to not only successfully apply manual techniques in specific positions, but devise specific home programs to enhance the progression of their patients.

During this course, the participant will participate in guided discussions related to the current evidence and research into Neuropathic Pain Syndromes.

Breaks
Break food will not be provided during the course.  You are welcome to bring your own snacks if desired.  Lunch each day will be on your own.

Attendance
Audience:  PT, OT. 

Each participant is expected to attend all hours of instruction to receive a Certificate of Completion. Any exceptions to this policy must be pre-approved in writing by the IPA Directors.

Lab Clothing
Each participant's body must be accessible for observation and treatment. Sports bra or halter-top and shorts are appropriate for women. Shorts or swim trunks for men. Please refrain from wearing large or sharp jewelry. A sweat suit is advisable between demonstrations, as temperatures may vary in the room.
